Membership List - "Opt-Out" Option
Per the requirements of state legislation (Assembly Bill 1098) enacted on July 1, 2006, residents have the right to opt-out of having their names included in membership lists that residents or their representatives can request for member-related purposes.  Residents can print this form to "Opt-out".
